How to carry out the procedure

• Once you access the website of the Superintendence of Health Servicesenter the tab Affidavit for Prepaid Users.
• To access the data upload form, click on the “new declaration” button. This option will only be visible from the 1st to the 20th of each month. One affidavit can be filed per hit and per period. From the 21st of each month onwards, the function will not be available.
• Click on the button “Selection of prepaid medical entity” to display the search engine that allows you to select a prepaid medicine company.
• The user must search for their service provider company through the phone number CUIT or entering part of the denomination, which will display a list of matches to look at. When the loan is found, it must be selected by clicking “accept”.
• Afterwards, you have to click on “Yeah” in the two condition boxes (declaration and authorization) and confirm the process by clicking the button “confirm”.

• The beneficiary may not register more than one Affidavit per period for the same prepaid. If you need to do it for more than one prepaid, it must be done through individual sworn statements.
• Once the process is confirmed, returns to the home screen with the registration of affidavits already entered.
